Sexual Health Coordinator Full Time

The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn. The Sexual Health Coordinator supports the implementation of sexual health services, including HIV/STI/HCV testing, prevention education, PrEP navigation, and linkage to care. This role serves as a critical bridge between the community and clinical services, ensuring patients receive timely, respectful, and comprehensive care.

The Coordinator plays a key role in patient engagement, outreach, documentation, and program support.

Essential Job Responsibilities And Duties

Duties and Responsibilities include but not limited to:

• Conduct rapid HIV, STI, and Hepatitis C testing in clinical and community settings.
• Provide pre- and post-test counseling and risk-reduction education.
• Support patient intake, screening, and navigation to appropriate services.
• Facilitate linkage to care for patients who test positive.
• Educate patients on PrEP and other prevention strategies.
• Support PrEP navigation, including referrals, appointments scheduling, and follow-up.
• Assist in retention efforts for prevention and care services.
• Participate in community outreach events and recruitment efforts.
• Engage priority populations in culturally responsive ways.
• Accurately document all services in EHR systems.
• Maintain timely and complete records in accordance with program requirements.
• Track service delivery, referrals, and outcomes.
• Assist with coordination of workshops, events, and group interventions.
• Maintain inventory of testing supplies and outreach materials.
• Support clinic flow and operations as needed.
• Adhere to all organizational policies and confidentiality standards (HIPAA).
• Follow NYSDOH and program-specific policies.

Education/Experience Requirements

• Bachelor’s Degree or equivalent experience
• Prior harm reduction, case management, and/or patient navigation experience
• Database management experience
• Exceptionally organized and able to work independently
• Compassionate and personable
• Willing to work in the field with community members
• Interest in health disparities
• Demonstrate ability to work effectively in a team environment
• Excellent ability to communicate orally and in writing
• Bilingual English Spanish speaking preferred
• Experience working with people living with HIV/AIDS, the LGBTQ community, and communities of color. Ability to communicate with the most vulnerable populations in Brooklyn and wider New York City area.

Other Related Skills/Experience And Requirements

• Knowledge of HIPAA rules in maintaining and ensuring patients/clients health information confidential
• Database management experience.
• Must have demonstrated ability to work independently and document all work performed.
• Understand the community’s cultures and ethnicities and knowledge of community assets and resources necessary to take advantage of its opportunities and to respond positively to its challenges.
• Knowledge of Brooklyn-based systems of care.
• Ability to deal effectively with a variety of people and work in a team environment.
• Sensitivity to the needs and situations of a multi-cultural population from a variety of income levels.
• Ability to prioritize goals and objectives to deliver mission-critical services, on time.
• Exceptionally organized and able to work independently.
• High level of accountability and initiative; needs to be a self-starter and eager to do team work to support the organization.
• Excellent interpersonal skills must demonstrate professionalism, courteous and respectful attitude in dealing with patients, team members, families and significant others
• Display courtesy, tact and patience with all members of the team members and extended community

Annual Salary: $50,000
