Cape Fox subsidiary, Eagle Health, LLC, is seeking a highly organized and motivated Social Worker to join our team in support of a federal government client. This role requires strong interpersonal skills, attention to detail, and the ability to work under tight deadlines while providing case management support, maintaining accurate case documentation, coordinating with supervisors and agency personnel, and supporting timely court documentation and child welfare case activities in accordance with applicable tribal procedures and requirements. This position is contingent upon the award.
Core Duties:
· Make decisions, in collaboration with a technical supervisor as identified by the agency, regarding service delivery to referrals of child endangerment and child protection
· Conduct interviews, assessments, and make home visits and collateral contacts with children and adults of all ages, as needed.
· Ensure all required documentation is completed (Application for Financial Assistance and Social Services, and Release of Confidentiality)
· Make professional decisions regarding current or impending danger and create effective interventions for the safety of children, up to and including possible removal
· Ensure required placement documentation is completed thoroughly (Placement Contract, Voucher, Placement Letter)
· Notify all parents/guardians, and supervisor or designee, as soon as possible if removal is necessary
· Identify, contact, and make transportation arrangements for placements if needed.
· Transport children for a medical screening with a local provider prior to initial placement
· File affidavits and petitions in tribal court (within 72 hours) when children are removed from their home and provide testimony in court hearings
· Coordinate with law enforcement, court staff, shelter providers and other human service agencies to meet client needs
· Participate in case planning and referrals for services as indicated by the assessment process and outcome. This may also include the transportation of adults or children
· Coordinate and provide direct services and activities that relate to social services, welfare assistance, Indian child welfare, adult protection services, and management of Individual Indian Account money holders (IIM)
· Child placement is made with a kinship/relative placement. ALL HOUSEHOLD MEMBERS aged 18 and over must have a thorough criminal and child welfare background check completed prior to placement
· Integrate all information and data points gathered via historical records check, reported information, direct investigation, and interviews into a comprehensive assessment of family functioning and needs, as well as identified child functioning and needs
· Provide a verbal summary of case activity/outcomes to the assigned supervisor or designee within 24 hours, or as otherwise agreed
· Submit case notes and a summary of activities to the supervisor or designee by email within 48 hours
· Maintain complete client contact information, including addresses and telephone numbers
· Maintain a case log that includes, at minimum: child name, date of birth, birth parent names, tribal court case number, placement name, placement address and telephone number, date of removal, and date the case is vacated
· Cloud-based Case Management System, to the extent possible, to document case activities
· Coordinate with the agency to obtain the required hardware, software, system access, training, and technical assistance
· Submit all required hard-copy case documentation, properly signed and executed, to the supervisor or designee within 48 hours
· Complete and submit all required court documentation to the supervisor for review within 48 hours of a removal, or as soon as possible, to allow corrections and final submission to the court within the required 72-hour window
· Coordinate with the agency’s designated technical supervisor regarding court timelines and ensure all documentation follows the Tribe’s Law and Order Code
Requirements:
· Minimum of a bachelor’s degree in social work or related field; however, a master’s degree in social work is preferred but not required or in a related field
· Must have a Background Verification
· Must have authorization to work in the United States as defined by the Immigration Reform Act of 1986
