What To Expect (Job Responsibilities)

• Create and track patients’ care plans while engaging regularly with them
• Monitor patients’ device readings and address any out-of-bound values
• Maintain meticulous records of patient interactions and program details
• Stay updated on the latest regulations and compliance standards related to CCM/RPM services
• Track care performance and strive to meet or exceed established goals

What Is Required (Qualifications)

• National Certification in Medical Assisting
• Residence in the U.S.
• Strong communication skills, both written and verbal
• Basic knowledge of chronic health conditions and their management
• Attention to detail and strong organizational skills

• Experience using technology for documentation and communication
