Role Description

This is a full-time remote role for a Patient Chronic Care Manager. The Patient Chronic Care Manager will support patients by coordinating chronic care management services, delivering quality patient care, and ensuring smooth transitions between healthcare settings. This role includes direct patient interaction, care plan development, and ongoing care coordination. Additional responsibilities include collaborating with healthcare providers to implement best practices and providing training as needed.

Qualifications

• Expertise in Patient Care, Quality Patient Care, and Direct Patient Care
• Experience in providing Training to patients and healthcare teams
• Nursing skills and a background in clinical care
• Strong communication and interpersonal skills
• Ability to work independently in a remote setting
• Proficiency in using healthcare technology and case management systems
• Understanding of chronic care management processes and best practices
• LPN or relevant healthcare certification preferred
