Nurse Case Manager Full Time

The Nurse provides clinical care, case management, and care coordination for individuals living with HIV as well as those seeking prevention services. This role focuses on improving health outcomes, increasing engagement in care, and reducing barriers to care that impact long-term wellness.

Education: Graduate of an accredited registered nursing program. Must be currently licensed in the State of Wisconsin. Preferred Education: Bachelor’s degree in nursing.

Experience: One year of general nursing experience in a clinic or hospital setting. HIV experiences preferred.

Expertise: Excellent clinical, critical-thinking, and interpersonal skills are essential. Strong oral and written communication skills are required. Effective organizational skills and computer literacy are critical. Ability to be self-governed and to work with diverse constituencies within the community, including health care professionals, community-based organizations, and governmental agencies.

Language: Comprehend and use basic language, either written or spoken, to communicate information and ideals.

Travel: A valid driver’s license and a reliable insured vehicle are required. Must be willing to travel locally and out of state.
