About the Opportunity: The Medication Navigator plays a critical role in supporting patients to adhere to their prescribed medication regimens. This position focuses on outreach and engagement to identify and overcome barriers to medication adherence, thereby contributing to improved patient experience and clinical quality measures. The role supports the organization”s quality initiatives by ensuring patients can effectively manage their medications.

Responsibilities:

• Engage patients, providers, and pharmacies to promote timely medication fills and refills, especially for chronic conditions.
• Educate patients on their medication regimens and benefits.
• Serve as a subject matter expert on pharmacy-related quality measures.
• Resolve issues and barriers to medication access, including cost reduction and mail delivery options.
• Achieve daily outreach goals through telephonic and email communication.
• Assist in developing programs and evolving workflows for high-risk patient management.
• Track and improve performance metrics and related initiatives.
• Work independently and coordinate activities, acting as a resource within the team.
• Identify and resolve complex problems related to medication adherence.
• Communicate with leadership to escalate issues and support solutions.
• Perform other assigned duties.

Qualifications:

• High School Diploma or GED.
• Minimum one year of experience working with patients, preferably in medication reconciliation or management.
• Proficiency with Microsoft Office, including Word and Outlook.
• Availability to work Monday through Friday, 8:30 am to 5:00 pm EST.
• Access to a secure, quiet home workspace capable of protecting patient information.
• Residence in an area with approved high-speed internet.

Preferred Skills:

• Bilingual in English and Spanish.
• Certification as Pharmacy Technician, Medical Assistant, Nurse, or relevant degree in pharmacy or nursing.
• Knowledge of medical terminology.

Pay Range and Compensation Package: The pay range and compensation package for this role will be determined based on the candidate’s experience, skills, and other relevant factors.
