Job Overview

We are seeking a dedicated Clinical Intake Coordinator to join our team for an in-person office position. The ideal candidate will be responsible for coordinating admissions closely with patients and their families, collaborating with healthcare professionals, and ensuring the best possible outcomes for patients and families.

Responsibilities

– Coordinate and conduct initial intake screenings (including determining eligibility and collecting necessary information for enrollment)

– Coordinate with healthcare providers to ensure appropriate patient care

– Ensure compliance with program regulations and standards participate in quality improvement initiatives and maintain knowledge of current healthcare regulations and trends

– Provide education and support to patients and their families

– Collaborate with interdisciplinary teams to optimize patient outcomes

– Assist in discharge planning and follow-up care arrangements

Requirements

– Experience in home health care is preferred

– Strong background in patient care and discharge planning preferred

– Excellent communication and interpersonal skills

– Ability to work collaboratively in a fast-paced environment

– LVN or RN License is required
