Responsibilities

• Dispense prescription medications accurately using pharmacy software and electronic health record (EHR) systems while adhering to all regulatory standards.
• Conduct comprehensive patient assessments to determine appropriate medication therapy, including evaluating medical history, physiology, and anatomy knowledge.
• Provide clinical pharmacy services such as medication counseling, immunizations, pain management advice, and patient education on medication administration and potential side effects.
• Manage inventory levels efficiently, including compounding medications and handling aseptic techniques for sterile preparations like IV infusions and infusions for critical care or pediatrics.
• Collaborate with medical teams on complex cases involving medical management, hospital protocols, and acute care procedures to optimize patient outcomes.
• Ensure compliance with HIPAA regulations by maintaining patient confidentiality and safeguarding sensitive health information across EMR/EHR systems.
• Assist pharmacy technicians with workflow coordination, inventory management, and maintaining accurate documentation of all pharmaceutical activities.

Qualifications

• Valid pharmacist license with current registration in the relevant jurisdiction.
• Proven experience in LTC (Long-Term Care) pharmacy or hospital settings, especially with critical care or acute care environments.
• Strong knowledge of medical terminology, physiology, anatomy, and pharmacology principles related to medication therapy management.
• Hands-on experience with pharmacy software systems, EMR/EHR platforms, and inventory management tools.
• Skilled in compounding medications using aseptic techniques; familiarity with IV insertion and infusion procedures is highly desirable.
• Experience in immunizations administration and patient service roles; certification in immunizations is preferred.
• Demonstrated ability to perform patient assessments, medication reviews, and collaborate effectively within multidisciplinary teams.
