How you’ll make an impact in this role

Communicate discharge recommendations of the clinical team to patient, care givers and/orinterdisciplinary team.

Responsibilities

• Participate in care planning and communicate information between patient and team. Document plan in medical record.
• Gather and arrange the necessary materials to enable patient discharge.
• Order supplies/equipment for patient specific needs as identified by clinical team.
• Perform administrative and clerical duties in support of unit/department.

Education

What minimum requirements you’ll need

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
