As a Registered Nurse Case Manager (RNCM), you’ll make a meaningful impact by delivering one‑on‑one, home‑based care that helps patients spend quality time with loved ones in the comfort of home. You’ll work at the top of your license, using clinical judgment and leadership skills to coordinate care, support families, and guide patients through their recovery. You’ll collaborate with a compassionate interdisciplinary team focused on excellent, patient‑centered outcomes.

How You’ll Work
Lead patient assessments and plans of careProvide case management and clinical care under physician guidance
Educate patients and families to promote safety and understanding
Work independently while managing time and visit priorities
Practice within state nursing scope and regulatory standards
Key Responsibilities
Conduct patient assessments and OASIS evaluations (training provided)
Develop and update individualized care plans
Administer medications and prioritize visits to reduce rehospitalizations
Supervise LPNs and nurse aides as assigned
Educate and advocate for patients and families using teach‑back methods
Complete documentation in compliance with agency, Medicare, and federal guidelines
Skills & Requirements
Compassionate, organized, and detail‑oriented
Comfortable working autonomously in home care
Active RN license in the state of employment
Minimum 1 year RN experience (acute care & home care experience preferred)
Valid driver’s license and active auto insurance
Pay: $65,250.00 – $82,275.00 per year
