Position Description
The primary responsibilities of the Path Forward Program Advisor are performing intake assessments with clients, determining goals and action steps, coordinating information, connecting clients with Love Columbia’s programs, making appropriate referrals to community programs and services, and providing clients ongoing case management and support from a strength-based, person-centered perspective. Advisors report directly to the Path Forward Program Manager.

Requirements:
· Master’s degree in Social Work is preferred however a Bachelor’s degree in Social Work or other relevant human services degree may be considered
· 2-3 years of case management or social work field experience strongly preferred
· Experience in relevant human services, such as counseling or education, may be considered
· Strong organizational skills
· Excellent problem-solving skills
· Capable of prioritizing tasks and handling multiple projects
· Proficient computer skills, preferably with database experience
· Must have a strong passion to serve marginalized populations
· Awareness of the complex issues faced by under-resourced individuals
· Exceptional interpersonal skills and the ability to listen to client stories and respond to potentially stressful situations with empathy, patience, and kindness
· Must have the ability to see potential for long-term development in all clients
· Commitment to Love Columbia’s mission, values, and work
Essential functions and duties:
·Intakes and Case Management
o Meet with clients in person or over the phone to gather required information for our intake assessment
o Listen to clients’ stories and gain an understanding of the root needs
o Ensure prompt and accurate entry of data into the client database
o Ensure an action plan with identified goals has been created for each new client
o Provide appropriate information and referrals to Love Columbia programs and community programs, to help clients accomplish goals
o Advocate for clients when needed
o Build positive relationships with clients
o Obtain the required documentation for consideration of financial assistance
o Provide timely and appropriate follow-up for each client, documenting all communication and progress in the client database
o Other Duties that pertain to case management and assisting the client with goal attainment.
· Coordinate and Supervise
o Coordinate with other Love Columbia programs to create a team that provides ongoing support to program participants
o Provide direct supervision of interns and volunteers, as assigned by the Path Forward Program Manager
· Connect with Community
o Refer clients to community agencies and churches to meet their needs and help them achieve their goals
o Provide feedback to referring agencies/churches as appropriate
o Collaborate with partner churches and social service agencies as needed

Physical Requirements
This position operates in a professional office environment requiring the following physical abilities:
· Ability to remain in a stationary position (sitting or standing) for extended periods of time.
· Ability to operate a computer, keyboard, telephone, and standard office equipment.
· Ability to communicate clearly via telephone, video conferencing, and in both written and spoken formats.
· Ability to lift and carry office supplies and materials weighing up to 20 pounds on an occasional basis.

Compensation and Benefits
This is a full-time position. Compensation is commensurate with experience. Health insurance benefits offered.
Pay: $45,000.00 – $50,000.00 per year
