Job Summary
We ae looking for MDS Coordinator with possiblity of 1-2 remote per week in various locations across RI. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within skilled nursing, rehabilitation, or long-term care settings. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, supporting compliance with regulatory standards such as RAI and CMS guidelines. By coordinating closely with interdisciplinary teams, you will facilitate optimal patient outcomes, streamline clinical documentation, and enhance quality of care delivery.
