Engagement Specialist

We are seeking compassionate, outgoing, and community-oriented Engagement Specialists who genuinely enjoy building relationships and supporting older adults.
This is a field-based, high-impact role for someone who thrives in a fast-paced environment, enjoys building relationships, and is passionate about serving seniors and underserved communities.
What You’ll Do
• Build trusted relationships with older adult patients and community members
• Maintain a visible and consistent presence within assigned communities
• Support patients throughout their care journey, including outreach, scheduling, enrollment, and follow-up
• Educate patients on Matter Health’s care model and help guide them through services
• Conduct outreach through in-person visits, phone calls, and community events
• Partner closely with care teams to support patient needs and ongoing engagement
• Help identify barriers to care and support patients with empathy and compassion
• Track engagement activity and follow-through within Salesforce/CRM systems
What We’re Looking For
• 1–2 years of experience in community outreach, healthcare, customer service, hospitality, sales, or other relationship-focused roles
• Strong communication and trust-building skills
• Compassion for seniors and underserved populations
• Organized, proactive, and comfortable in a fast-moving environment
• Valid driver’s license and reliable transportation required
• Salesforce, healthcare, Medicare, or community engagement experience is a plus
• Bilingual Spanish is a plus
