Job Title: Community Health Worker
Salary: $21.63/hour (Paraprofessional 1)
Work Schedule: 40-hour work week
Location: This is a Louisiana-based, hybrid position with regional (80%) travel required in Central Louisiana
Team: Capacity Building
Reports To: Senior Manager, Capacity Building

About The Position
A successful candidate will have a strong connection with and regional expertise of priority communities in Louisiana. The candidate should be an independent, self-starter with strong abilities to manage multiple responsibilities and prioritize appropriately, thrive in a collaborative environment, proactively anticipate, and manage challenges. Day-to-day responsibilities may include managing schedules and patient appointments, working directly and communicating with patients in a community or clinical setting, working closely with clinical staff, managing referrals, identifying, and prioritizing items for attention, documentation management, and program activity coordination.
