Case Manager / Social Worker Exp with Substance Abuse Full Time NEW

Role &Responsibilities:

• Communicate with SCI MAT Program Specialists regarding reentrants experiencing overdose or returning for detox to determine MAT eligibility and appropriateness.
• Assist with MAT referrals in the community by scheduling medication and treatment appointments.
• Ensure individuals receive required services and benefits in alignment with their treatment plans.
• Track, update, and maintain accurate databases/spreadsheets for individuals released on MAT programs.
• Attend required meetings, including weekly Vivitrol conference calls and interdisciplinary team meetings.
• Document all client interactions in systems such as Captor, Sapphire, and other required platforms.
• Conduct assessments for individuals with positive drug screens and refer them to appropriate treatment services.
• Complete required GPRA assessments (initial, 6-month, and discharge evaluations).
• Meet with reentrants in correctional facilities, parole offices, and treatment centers as needed.
• Communicate treatment plans and updates with staff, parole officers, and clinical teams.
• Provide counseling, case management, and social work services to support rehabilitation and social functioning.
• Assess psychosocial needs, strengths, and eligibility for services using professional social work practices.
• Apply knowledge of counseling techniques, behavioral health, and treatment interventions.
• Ensure compliance with HIPAA, confidentiality laws, and correctional health policies.
• Utilize Microsoft Excel for tracking, reporting, and case management documentation.
• Maintain strong communication, collaboration, and documentation standards in all professional interactions.
