Job Description:

Supports the clinet in managing and monitoring children with elevated blood lead levels (EBLL) across an assigned 20–40 county region. Ensures timely case follow-up, compliance with surveillance requirements, and coordination between local health departments and state programs to reduce lead exposure risks.

Key Responsibilities:

• Manage and track EBLL cases in NBS, ensuring timely follow-up and closure

• Monitor compliance with case management timelines and reporting requirements

• Coordinate with IDOH teams, data analysts, and local health departments

• Identify trends and support environmental risk assessments

• Provide technical guidance on lead regulations and case procedures

• Conduct periodic case reviews, reporting, and stakeholder meetings

• Engage with families when needed for follow-ups, retesting, or home visits

• Support documentation, program materials, and grant-related work

• Travel within assigned counties for coordination and field support

Qualifications:

• Proficiency in MS Office, Adobe, and virtual meeting tools

• Strong case management, organizational, and communication skills

• Ability to manage multiple cases and adapt to changing priorities

• Sound judgment handling confidential health information

• Basic understanding of data systems and case tracking tools

Preferred:

• Bachelor’s in public health, nursing, environmental health, social work, or related field

• 2+ years in case management or public health programs

• Experience with lead exposure/healthy homes programs

• Familiarity with surveillance systems and Indiana lead regulations (410 IAC 29)
