Job Overview: As a Healthcare Liaison Officer, you will play a pivotal role in strengthening relationships between healthcare institutions such as hospitals and the communities they serve. Your primary objective will be to engage with community members, local hospitals, to promote our facility, and participation in healthcare initiatives. This role requires a blend of communication skills, strategic thinking, and a genuine commitment to improving health outcomes at the community level.

Key Responsibilities:

• Community Engagement: Develop and implement strategies to engage with diverse community groups. Foster trust and collaboration by actively listening to community needs and concerns.
• Partnership Development: Cultivate partnerships with local organizations, schools, hospitals, faith-based institutions, and grassroots community groups to leverage resources and expand the reach of healthcare initiatives. Identify opportunities for collaboration and joint programming to address community health priorities.
• Resource Referral Services: Serve as a resource referral hub for community members seeking information about healthcare services, insurance options, and social support programs. Provide guidance and assistance in navigating the healthcare system and connecting individuals to appropriate resources.
• Evaluation and Impact Assessment: Monitor and evaluate the effectiveness of community outreach efforts, using both quantitative and qualitative methods to assess reach, engagement, and outcomes. Identify lessons learned and best practices to inform future program planning and continuous improvement.

The goal of this position is to be out at the hospitals and the community to bring the facility patient referrals.
