Pharmacist
Reports To: Pharmacy Director
Status: Salaried, Exempt

POSITION SUMMARY
The role of this position is to assist the Pharmacy Director in the
daily operations, management of staff, evaluation, optimization and
promotion of pharmacy services designed to provide value from improved
patient outcomes and financial return through available revenue
channels. Organizes and directs the workflow of operations within the
department according to Katahdin Valley Health Center (KVHC) and
Pharmacy policies, procedures, mission, goals and values. Provides
direction for technician staff regarding medication management
processes, including financial stewardship. Implements optimal systems
of care in coordination with the Pharmacy Director, solves problems and
facilitates processes required to achieve health system strategic
objectives within budgetary targets. Additional responsibilities include
management of assigned staff, quality measurement and process
improvement, practice standards within area of responsibility and
regulatory compliance.
RESPONSIBILITIES
Supervision
Provides direct supervision and direction of Pharmacy Technicians.
Provides clear oversight and direction for the Pharmacy Technicians
in such a way that all professional and technical activities performed
are consistent with the KVHC mission, core values, supporting behaviors
and policies.
Operations Management
Fills prescriptions in an accurate and timely manner. Reviews and
verifies or enters orders into the medication management information
system. Assures that orders present no significant problems in regard to
drug interactions, incompatibilities, therapeutic duplications or
patient safety.
Supervises and directs work of pharmacy technicians. Coordinates
medication preparation and distribution work of the pharmacy technicians
and ensures accuracy prior to dispensing medications for distribution.
Provides counseling to patients, families, or caregivers regarding
their medications, to support appropriate and effective drug use.
Provides accurate, appropriate and timely information to physicians,
nurses, patients and others regarding medication therapy, drug product
guidelines, law and regulations, medication formulary and other topics
related to medication use and control.
Supervises the day-to-day operations of the pharmacy that include
inventory management, interactions with insurance companies, and
accurate and appropriate record keeping.
Consults with prescribing practitioners to help ensure accurate and
appropriate medication use. Reviews medication orders and evaluates
their appropriateness and completeness. Contacts theprescriber with
suggestions for improvement and resolves problems with the medication
orders as needed. 6. Monitors daily activities as they relate to the
procurement, storage, preparation, distribution, compounding,
monitoring, control and dispensing of medications and supplies. Ensures
that there is a mechanism for accountability for these tasks. Evaluates
existing service levels, resources and staffing and makes
recommendations to the Pharmacy Director to ensure that service needs of
all customers are met.
Manages the daily operations of the pharmacy as it relates to
staffing, medication procurement, indigent care, dispensing, and patient
counseling, coordination of selected reimbursement scenarios and
determination of patient eligibility for service.
Manages the non-sterile product preparation process and ensures that
adequate controls and standards are maintained in order to safeguard the
patient, care provider and employee.
Actively supervises controlled substance procurement, documentation,
storage, distribution, disposition and usage audits.
Comply with the 340B program requirements and all related
operational elements to ensure program integrity and optimization in
coordination with the Director of Development / Grants Management and
Pharmacy Director.
Clinical Roles
Participates in pharmacy services through collaboration with
physicians and other health care providers in the management of
medication therapy in the ambulatory care patient population, including
direct patient care pharmacotherapy management, provision of drug
information and practice support.
Participates in physician, team and staff education and training for
integrated care; continuing education about integration and
evidence-based practice is routinely completed to maintain knowledge and
skills.
Provide Medication Therapy Management (MTM) services and associated
clinical pharmacy services to optimize drug therapy and improve
therapeutic outcomes for KVHC patients by identifying, preventing and
resolving medication-related problems.
Administer immunizations per 32 MRSA 13831-13835, according to and in
compliance with the Pharmacy Rule Chapter 4-A, as authorized by KVHC
Standing Order Protocol
