As a Children’s 1st Case Manager / Non-licensed social worker within East Central Public Health District, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Starting Salary: $40,000.00

Job Responsibilities

Under general supervision provides a broad range of social services and/or community health services for infants, children and families within the East Central Health District.

Provide developmental screening (Ages & Stages; MCHAT-R) to all children, birth to age

five years, as assigned by District Children 1st Coordinator.

Provide home visits to include assessments, parent education, developmental screening and activities according to Children 1st model of service delivery.

Refer infants and children to public health (BCW, CMS, EHDI, WIC, etc.) and community programs as appropriate.

Ensure that all children receiving services are linked to a primary healthcare provider.

Complete all necessary documentation, both written and electronic, and reporting requirements by specified deadline

Assist with data entry, scanning & filing as needed and/or assigned.

Promote all DPH Child Health programs through community engagement and events.

All East Central Health District 6 employees are deemed essential workers in the event of an emergency. As such, you may be called on to participate in an event including but not limited to: staffing a shelter, working at a Specimen Point of Collection Site (SPOC) and/or working at a Point of Dispensing site (POD). If you are called on to participate, you are required to report to the designated site as a condition of employment, whether the emergency occurs during or outside regular work hours.

Minimum Qualifications

Bachelor’s degree in a related field from an accredited college or university and two

years of experience in social service delivery.

Public Health and home visiting experience

DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree, diploma, licensure, etc.
