About us
We are seeking Physical Therapists to join our team of home health professionals. The successful candidate will be responsible for providing quality care to patients in their homes, assessing and monitoring patient conditions, and providing education and support to patients and their families. The ideal candidate for this position will have excellent communication skills, an ability to work independently, and a commitment to providing the highest level of care.
Essential Functions:
As a Physical Therapist, you will manage and provide a wide range of physical therapy services to patients in the home setting. Seeing the end results of their recovery is just one of the many rewards of the position! Your primary focus will be:
- Patient assessment
- Delivering quality care
- Monitoring of patient status and compliance
- Teaching of patients and their families to achieve maximum patient independence in accordance with functional level and potential
- Discharge planning
Requirements:
- Minimum Bachelor’s degree from an accredited Physical Therapy program
- Active Physical Therapy licensure in the state of Illinois
- Possess a valid driver’s license and current auto insurance
- Previous experience in home care setting is desired
- In-depth, up-to-date knowledge of physical therapy, theory and practice and the legal and ethical issues associated with them

We currently have openings in the following territories;
- South Side
- South Suburb
Pay: $90,000.00 – $110,000.00 per year
