We are seeking an organized, compassionate, and detail-oriented Palliative Care Coordinator to join our growing team. This position plays a key role in coordinating patient care, scheduling services, processing referrals, obtaining authorizations, and supporting our interdisciplinary care team.
Schedule: Monday – Friday | 9:00 AM – 6:00 PM
$21.00 – $25.00/hour (DOE)
Spanish speaking preferred
What We’re Looking For
- Strong computer and customer service skills
- Excellent communication and organizational abilities
- Hospice, home health, or palliative care experience preferred
- Ability to multitask in a fast-paced healthcare environment
- Professional, compassionate, and team-oriented attitude
- Valid driver’s license and reliable transportation
Key Responsibilities
- Coordinate palliative care referrals and admissions
- Obtain insurance authorizations and re-authorizations
- Schedule patient visits and coordinate care services
- Communicate with physicians, nurses, social workers, and care teams
- Review documentation for accuracy and timely submission
- Support patient and family needs while ensuring quality care delivery
- Maintain accurate patient records and reports
